TREATMENT OF CEREBRAL VASCULAR DISEASE


Abstract
In most countries cerebral vascular disease is the third, and in Korea first leading cause of death. The various measures available for the treatment of a patient with cerebral vascular disease have been discussed, and the value of vigorous conservative treatment has been emphasized.
During acute stage the treatment is directed toward maintaining life and preventing of the development of complications.
Bed rest, administration of a sedative if need, position change to prevent the hypostatic pneumonia or decubitus, maintenance of adequate airway, administration of oxygen, urinary catheter, prevention and treatment of infection are important conservative treatment in acute stage. Convulsion is controlled with Dilantin, and it is wise to avoid use of barbiturates because of their depressing effect on respiration.
The value of anticoagulants in cerebral thrombosis is no clear as yet. The role of anticoagulants in treatment of cerebral embolism seems well established.
Hypothermia is inconclusive in the treatment of cerebral vascular discase.
Recently, hyperbaric oxygenation therapy may provide supportive therapy in some patient with acute cerebral ischemia.
In recovery stage the rehabilitation of the patient with paralysis due to cerebral vascular disease should be started early and should be intensive.
